Occupational Summary
Provide initial assessment diagnosis, evaluation and treatment for EIC Clinic patients. Provide individual group therapy. Serves as a liaison for Duke Health Inequalities Program to provide client centered services to integrate substance abuse/mental health treatment to HIV infected patients through case management, outreach, active coordination of care between community based providers (case managers) as well as the ID clinic providers. The CSW/SAC ensures timely and coordinated access to medically appropriate levels of health and support services and continuity of care, through ongoing assessment of the client’s and other key family member’s needs and personal, support systems. This position requires collaboration with all members of the CADET grant, including sharing of clinical leadership of 2 group treatment sessions per week at the Duke Addictions Program. Actively supports the Lincoln Community Health Center’s mission.

Qualifications
A general knowledge of Durham community and its resources is required. Knowledge of HIV/AIDS through seminar and course work required. Interviewing, facilitation, and computer skills required. Cultural competence through ability to communicate effectively, verbally, and in writing to diverse population including patients, staff, and general public required. Ability to perform with minimal supervision desired.

MSW or masters degree in a related field required. LCSW and LCAS preferred. Some knowledge or previous work experience in the health field especially to HIV/AIDS care. One year of working in a community /public health setting.
